In healthcare the need for continuous, real-time patient monitoring doesn’t end at the hospital discharge. RPM or Remote Patient Monitoring is designed to keep track of patients as they leave hospital wards. It ensures that clinicians can continue to observe critical health metrics and respond promptly if conditions change.

We developed an end-to-end RPM solution designed to meet this exact need—delivering consistent monitoring, actionable insights, and peace of mind for patients, families, and healthcare providers.

What We Did

Our approach focused on creating a fully integrated system that handles the entire remote monitoring lifecycle, from data collection to decision-making. The solution includes:

  • Real-time patient monitoring
  • Tracking heart rate and other vital signs
  • Advanced dashboard analytics for clinical insights

Our Solution

We created a comprehensive RPM solution that helps healthcare providers monitor patient health remotely using advanced sensor technology and a seamless digital infrastructure.

Key Capabilities:

  • Real-time updates on patient health data
  • Secure data transmission from device to cloud
  • Scalable architecture that supports multiple patients simultaneously
  • Customizable features based on provider requirements

Our system integrates hardware and software components to create a user-friendly experience across patient, family, and clinician touchpoints.

Sensor and Firmware Development

The Central part of RPM system is our own proprietary sensor hardware, designed and developed by us. This sensor has been integral in collecting real-time health data continuously, without requiring any manual input from the patient.

Features of the Sensor:

  • Bluetooth connectivity for seamless pairing with mobile and gateway devices
  • Custom firmware that manages low-power operation and accurate data transmission
  • Compact design for easy wearability and patient comfort
  • Consistent performance under different environmental and physiological conditions

The sensor automatically transfers data to a local gateway device, which forwards it to secure servers for further processing.

Mobile Application for Patients and Family Members

We have developed a mobile application specifically intended for patients and their families so as to encourage patient engagement and transparency. It is a vital component in visualizing and understanding the health data of users.

App Features:

  • Live health stats including heart rate and vital trends
  • Visual charts to track improvements or declines over time
  • Secure access for both patients and family members
  • User-friendly interface with intuitive navigation and alerts
  • Health milestones and reminders for medications or activities

This app helps families stay involved and gives patients more control over their own recovery journey.

Dashboard for Doctors and Hospital Staff

We developed a browser-based dashboard tailored to the needs of clinicians and hospital staff. This tool provides a real-time view into each patient’s health data, enabling fast decision-making and better outcomes.

Dashboard Highlights:

  • Centralized patient data in a clean and structured layout
  • Advanced filtering for symptom-specific analysis
  • Custom alerts triggered by changes in vital signs
  • Easy export of reports for documentation or follow-up
  • Role-based access for staff to ensure data privacy and control

Physicians and nurses can use this dashboard to remotely evaluate patient conditions, adjust care plans, and intervene when necessary, all without needing to bring the patient back to the clinic.

Remote Monitoring Functionality

At its core, this project solves the need for continuous health tracking in the critical recovery window following hospital discharge. Our RPM system delivers on that promise through real-time monitoring and reliable connectivity.

Benefits of Our RPM Implementation:

  • Continuous data collection without disrupting the patient’s daily routine
  • Faster detection of medical anomalies or deterioration
  • Increased clinical efficiency with less reliance on in-person visits
  • Reduced readmission rates through proactive care
  • Increased the communication between patients and healthcare providers.

This features is of particular importance for patients suffering from chronic diseases, recovering from surgical procedures, or aged individuals that may require continued surveillance.

Data Collection

The effectiveness of any RPM solution depends on the quality of data it gathers. We built our system with data reliability and completeness at the forefront.

How Data is Collected and Managed:

  • Continuous sampling of heart rate and vitals via the wearable sensor
  • Automats transmission from the sensor to cloud storage via gateway
  • Encrypts storage of all data on secure, compliant servers
  • Access logs and audit trails to track how data is used
  • Data visualization for clinicians to make informed decisions quickly

Our goal was to ensure healthcare professionals have all the information they need, without the noise, delay, or uncertainty that can come with manual inputs.

CE Certification Assistance

To bring this solution closer to market readiness, we also guide the certification process. Medical-grade technology must comply with stringent regulatory frameworks, and our involvement helps streamline this step.

Our Role in the Certification Process:

  • Preparation of technical documentation as per CE standards
  • Support for clinical evaluation requirements
  • Assistance with risk assessment and safety protocols
  • Alignment with European health, safety, and environmental regulations
  • Ongoing support during the certification review process

Our support enabled faster and smoother navigation through CE marking requirements, making the product safer and more acceptable for wider deployment.

Conclusion

Remote patient monitoring is no longer a luxury, it’s a necessity in modern healthcare. By combining in-house hardware development, intuitive software design, secure data practices, and regulatory guidance, we created a single unified system that addresses the real-world challenges of post-discharge patient care.

This project is a step toward more proactive, patient-centered healthcare—keeping patients safe, connected, and cared for, even after they leave the hospital.
